Student Health
Chaminade University supports a healthy and safe campus environment in accordance wit the Hawai 'i state law. Students enrolled in in-person courses must meet all health clearance requirements before attending classes, residing in campus housing, or participating in athletics. Students enrolled exclusively online are exempt.
Chaminade does not operate an on-campus health center, however, free transporation is available for residential students to nearby clinics and hospitals. Students are also encouraged to follow CDC adult immunization recommendations, https://www.cdc.gov/vaccines/adults/rec-vac/index.html.
Required Documentation
All health records must be submitted through Med+Proctor.
Create a Med+Proctor account using your Chaminade email and Student ID number.
The service is free - do not pay for expedited services.
Records must include the student's full name and date of birth for identification purposes
Students must submit:
TB Clearance
MMR - 2 doses
Tdap - 1 dose
Varicella - 2 doses
MCV - Required for first-time residential students age 21 and under
Health Insurance Verification - Required for residential students, student-athletes, and undergraduate international students.
International students must provide immunization records in English and verified by a U.S.-licensed healthcare profider.
